The most common reason for this behavior is not checking the QuickBooks option to 'Print Company Name and Address on check.'
This option is on the QuickBooks\File\Printer Setup\Check|Paycheck setup screen.

cant get pb to recognize seperate banks puts info from one bank on the other check from other bank
